Before there was photoshop, you had to do this in the camera. WIth multiple exposures. This is a lmage I did back in 1979,when I was playing around with images similar to a photographer named Jerry Uelsmann , whose work I admire. If you love surreal images it would be worth your while to check out his web site. He is world renown and is an instructor at Gainsville Florida College.
