I think it is the choice of clothing that makes the woman's head look small in proportion to her body; the wide sleeves and shoulders of the blouse.

Your detail is wonderful but you might work on your proportion (head to body size.) If you stand back and use your thumb to see reference points it will be easier for you to be more accurate keep on keeping on
